for Parbawatiya Kadampa Buddhist Center, DBA Kadampa Meditation Center Tampa BayI the undersigned declare that:
- I am a practitioner of Buddhism within the New Kadampa Tradition; and
- I do cherish the Center and the New Kadampa Tradition; and
- I do practice good moral discipline; and
- I am a student of any of the Center programs and have been so for at least one year; and
- I do accept the Internal Rules of the New Kadampa Tradition; and
- I am at least eighteen years old; and
- I am free from any conflict of interest including, but not limited to, being associated with or practicing in a spiritual tradition other than the New Kadampa Tradition.
